As a new patient, you may be wondering what to bring to your consultation and what to expect when making financial arrangements. Here are some tips to help you prepare for that first visit.


What to Bring for your Appointment

We encourage patients to bring the following items on their first visit:

  • Driver’s License or State ID Card
  • Insurance Card
  • Social Security Card
  • Cash, Check, MasterCard, or Visa for any co-payment
  • Recent medical or oral health records from other offices
  • List of medications you are taking
  • List of questions or concerns about dental health or treatment

What to Wear

Your first visit typically includes X-rays, an oral exam, and teeth cleaning. Please wear loose-fitting and comfortable clothing, as you will be seated in the dentist’s chair for a good portion of your first visit.

If you have anxiety or fear about being in the dentist’s chair, talk to Dr. Yu about sedation options. We are here to ensure every visit is as comfortable and stress-free as possible! Please don’t hesitate to share your fears or concerns at any time.

Forms

If you picked up a new patient form from our Bellevue dentistry office on a previous visit or have downloaded and printed the form from below, please bring that with you. Filling out these forms before you arrive will help us get you through your first visit quickly.

Payment

We encourage all patients to review and understand their benefits before proceeding with treatment. It is your responsibility to find out which services will be covered and not covered by your insurance policy. Your estimated co-pay and deductible will be due at the time of service.

For your convenience, we accept cash, check, MasterCard, and Visa credit cards as methods of payment. We will provide a receipt of payment and services for your records. Please be prepared to sign a Financial Policy Form when you arrive.

Please avoid eating a very heavy meal before your visit so that you are more comfortable during the oral exam and teeth cleaning session. You may brush and floss your teeth before arriving.
